A sharp output spike exactly when setpoint changes can come from derivative acting on error. Separate that response from a real process disturbance.

What is happening?
Error contains both setpoint and process value. A step in setpoint creates a rapid error change even if the physical process has not moved. Derivative on the measured process value responds differently; available weighting and filtering depend on the controller.
Check these five things

Trend setpoint, process value, and individual PID terms if available.

Check whether the derivative source is error or process value.

Look for an output impulse aligned with the setpoint step.

Worked example
In a teaching loop, setpoint steps from 40 to 45 while the measured value remains 40 at that instant. The sudden error change can produce a derivative contribution without a sudden physical temperature change. A separate disturbance trial reveals the distinction.
